Dr.1010beginswithanblank10x10gridofsquaresandthreecolored,irregularlyshapedfiguresatthebottomofthescreen.Thesefiguresareanarrangementofblocksthesamesizeastheblanksquaresinthe10x10grid.Theplayermustdragtheseblocksontothegrid,andaftertheblocksaredropped,theyareunabletobemovedfromwheretheywereplaced.Assoonasthethreefigureswereplacedonthegrid,threenewfiguresappeartobeplaced.Thiscontinuesforaslongasthegameisbeingplayed.
Oncearoworacolumnof10onthegridisfilledentirelywithblocks,thatroworcolumnisclearedofblocksandappearsblankagain.Asthegameisplayed,therearelessandlesspossiblespacestoplaceblocks,soclearingrowsandcolumnsistheonlywaytogainareabackontheboard.
Thegoalofthisgameistogetasmanypointsaspossible.Thepointsforeachblockplacedontheboardcorrespondstothenumberofsquaresthattheblockcovers.Blocksrangefrom1to5squares.
