Theseguidelineshavenotundergoneaformalprocessofevidencebasedclinicalpracticeguidelinedevelopmenthowevertheyaretheresultofconsensusopiniondeterminedbytheexpertclinicianworkinggroup.Theyarenotadefinitivestatementonthecorrectproceduresrathertheyconstituteageneralguidetobefollowedsubjecttotheclinician’sjudgmentineachcase.Theconsensusopinionprovidedisbasedonthebestinformationavailableatthetimeofwriting.
